抗菌骨科植入物的现状及发展趋势

      Current Status and Development Trends of Antibacterial Orthopedic Implants

      • 摘要: 随着骨科植入物应用的日益广泛,植入物相关感染已成为临床面临的严峻挑战。为降低感染风险,局部抗菌策略应运而生,通过加载抗生素、银、碘、季铵盐等物质实现植入物局部抗菌。现有研究表明,部分涂层已在临床试验应用中展现出抗感染潜力,尤其在骨肿瘤等高风险患者中具有积极意义。然而,不同涂层的安全性表现各异,银涂层可能引发银质沉着症等局部反应,而抗生素涂层存在潜在耐药风险。本文就含抗菌涂层骨科植入物的技术发展、临床应用现状及安全性问题进行综述,以期为推动该技术在骨科感染防控中的规范化应用与优化提供参考。

         

        Abstract: With the increasing application of orthopedic implants, implant-associated infections have become a critical challenge in clinical practice. To mitigate infection risks, local antimicrobial strategies have emerged, which involve loading implants with substances such as antibiotics, silver, iodine, and quaternary ammonium compounds to achieve localized antibacterial effects. Existing research indicates that certain coatings have demonstrated potential in reducing infections in clinical trials, showing particular promise in high-risk patients, such as those with bone tumors. However, the safety profiles of different coatings vary. Silver coatings may lead to localized reactions like argyria, while antibiotic coatings carry potential risks of drug resistance. This paper reviews the technological advancements, current clinical applications, and safety concerns of antimicrobial-coated orthopedic implants, aiming to provide insights for promoting standardized and optimized use of this technology in preventing and controlling orthopedic infections.
