锥形束CT图像用于盆腔肿瘤剂量计算准确性评估

      Accuracy Assessment of Cone-Beam CT Images for Pelvic Tumor Dose Calculation

        目的 评估锥形束CT(cone-beam CT, CBCT)图像在盆腔肿瘤放疗剂量计算中的可行性和精确度。
        方法 改进容积密度覆盖法建立CBCT图像的CT值(hounsfield unit, HU)-相对电子密度(relative electron density, RED)曲线,将定位CT计划移植至CBCT图像上,在优化参数不变的情况下,CBCT计划使用构建的密度曲线计算剂量,比较两种计划的剂量计算偏差。
        结果 两种计划的靶区和危及器官各项剂量学参数平均差异分别小于1%和1.5%,靶区适形性指数(conformity index, CI)、均匀性指数(homogeneity index, HI)以及伽马通过率均较为接近,差异均无统计学意义。
        结论 经该方法修正后的CBCT图像可用于盆腔肿瘤放疗计划剂量计算。

         

        Abstract: Objective To evaluate the feasibility and accuracy of cone-beam CT (CBCT) images for radiotherapy dose calculation in pelvic tumors.
        Methods Improved volumetric density cover method to establish CT value-relative electron density curves of CBCT images, the localization CT plan was transferred to the CBCT images, and the constructed density curve was applied to calculate the dose of CBCT plan while maintaining the optimization parameters unchanged, and the deviation of the dose calculation between the two plans was analyzed.
        Results The mean differences in dosimetric parameters between the two plans in the target area and organs at risk were less than 1% and 1.5%, respectively, the target area conformity index (CI), homogeneity index (HI), and gamma passing rate were relatively close to each other, and the differences were not statistically significant.
        Conclusion CBCT images corrected by this method can be used for pelvic tumor radiotherapy plan dose calculation.
